The forms below are available for those who would like to download and print out the form for home use to be mailed to us. A mailing address is included with each form.
Special LLHF Membership Category: If you are a WWII Veteran and would like to become a member of Land of Lincoln Honor Flight, just click on the membership link above to download the blank application, complete it, and send it to us with NO MONEY - that's right, WWII vets join for free.
Honor Flight Applications will be processed in the order they are received. An exception will be made only with a doctor's written notice stating terminal illness is an issue to be considered in the priority of the flight assignment.
Description of our Hub Boundaries: (see our hub map here)
- At Alton, follow Route 140 to I55 (includes Alton). Follow I55 north (includes Mt. Olive) to Litchfield.
- Follow Highway 16 East and then Northeast to and including Rosamond. Go North following Highway 7 to Highway 48 including Owaneco.
- Follow Highway 48 to bypass around Decatur. Follow this bypass around the western and northern edges of Decatur to exit 156.
- Follow Highway 48 North up to I55. Include LeRoy, Fletcher, Pleasant Hill and Lexington. Do not include Chenoa.
- Go west to include Gridley, El Paso, Roanoke, Metamora, and Spring Bay. Cross the Illinois River and go around Peoria to Highway 24 to include Mossville, Alta, Norwood, Bellvue, Bartonville, Tuscarora as you go around Peoria. Follow Highway 24 to Quincy.
- Follow the state boundary back to Alton to complete our boundary.
As a Guardian, You Must:
- Complete a guardian application as soon as possible. If a veteran has requested you as a guardian, your guardian application must be sent in right away.
- Be responsible for the safety and comfort of one or two veterans at all times on the day of the trip. If you have requested to be a guardian for a family member or friend, you may be asked to be responsible for another veteran.
- Attend guardian training before the flight (lasts about 1 to 1.5 hours and is offered at different dates and locations). You will not be able to be a guardian if you do not attend this training.
- Pay the guardian fee at least a month before the flight. This fee covers guardian expenses (the plane ticket, bus transportation, three meals, shirt and hat). In 2009 that charge was $400. No money is accepted from the WWII veterans.
- Meet the veteran(s) before the flight to conduct an interview and complete forms by the dates given. Return the forms by the dates given.
- Communicate last-minute reminders and information to your veteran(s).
